They look scary, but they are easier to use than they appear and can be used to create a variety of beautiful gathered and pleated effects. A ruffler is more correctly a pleater, as it pushes a fold of fabric under the needle using a sliding bar. A ruffler can be used with a single layer of fabric to pleat or gather it, or it can be used to simultaneously pleat one layer of fabric while stitching it onto another layer.
